Shared Value Network: Establish a local and national network across patients, community leaders, providers, researchers and businesses, powered by an integrated knowledge system that prioritizes insight and value sharing aligned with cross-community needs.
Harness Insights from trusted and reliable data: Leverage the vast commercial healthcare medication adherence data from Komodo Health aligned with US Census demographic data at ZIP code levels to reveal where and how compounded social risk characteristics relate to medication adherence for the diseases of metabolic syndrome: hypertension, hyperlipidemia and diabetes.
Publicly Share Insights in CMU CREATE Lab’s EarthTime Data Visualization Tool: Utilize EarthTime's powerful data visualization capabilities to explore and reveal trends in community health and wealth, and share across communities to scale local best practices.
Community Cohorts: Insights and priorities from local and national community leaders, along with their stakeholders, will guide the direction of pilot roadmaps, including goals, strategies, decisions and execution.

Outcomes Details
Sustainable Partnerships: Trusting partnerships among community leaders, advocates, healthcare and service providers and their stakeholders, starting with local community leaders and their stakeholders in the Pittsburgh and Northeast Corridor regions.
Community-Driven Solutions: Facilitating the creation, prioritization, and execution of shared community goals through collaborative network efforts guided by data-driven insights from the shared value network.
Community-authored and data-informed narratives: Our champion community leaders and their stakeholders will draw on trusted data-informed insights. Together, they will co-author community-based narratives that reveal their lived experiences to create the best practice solutions that improve community health outcomes and improve stakeholder value.
Increased value for community members and their stakeholders: Community champions and cross-community stakeholders will define, evolve and report on their aligned specific, measurable, attainable, relevant and time-bound (SMART) goals.
A marketplace of clearly defined and trusted community level issues, best practices and shared resources who can help: Our work lends us to help refine problem statements - directly from local communities. Then we tie these issues to best practices, solutions, and innovators working to solve these problems. And lastly, we share these insights with other communities who may have similar issues.
